🩺 What You Need to Know

If you’ve been diagnosed with lymphedema and have been prescribed an Assistive Devices Program (ADP) application, here’s how to move forward:

✅ Step-by-Step Instructions:

  1. Visit a Doctor Registered with ADP Your application must be signed by a physician who is officially registered with the ADP program.
  2. Meet with a Lymphedema Nurse You’ll also need to sign the ADP application during your appointment with a certified lymphedema nurse.
  3. Submit the Application to Us Once your form is signed by both professionals:
    • 📧 Email the completed application to us - cfhhc@yahoo.com
    • 📬 Drop it off in person at 2396 Major Mackenzie drive West , Unit 4 , Maple , L6A 4Y1
  4. Final Submission & Approval After we receive your application:
    • We’ll carefully review and sign it
    • Then, we’ll submit it to the ADP for government approval
Once approved by ADP, we’ll receive a claim number on your behalf. This number is valid for two years and allows coverage for:
  • Compression garments and sleeves for chronic lymphedema
  • Sequential extremity pumps and accessories for primary lymphedema.
