🩺 What You Need to Know

If you’ve been diagnosed with lymphedema and have been prescribed an Assistive Devices Program (ADP) application, here’s how to move forward:

✅ Step-by-Step Instructions:

  1. Visit a Doctor Registered with ADP Your application must be signed by a physician who is officially registered with the ADP program.
  2. Meet with a Lymphedema Nurse You’ll also need to sign the ADP application during your appointment with a certified lymphedema nurse.
  3. Submit the Application to Us Once your form is signed by both professionals:

    • 📬 Drop it off in person at 2396 Major Mackenzie drive West , Unit 4 , Maple , L6A 4Y1
  4. Final Submission & Approval After we receive your application:
    • We’ll carefully review and sign it
    • Then, we’ll submit it to the ADP for government approval

Once approved by ADP, we’ll receive a claim number on your behalf. This number is valid for two years and allows coverage for:

  • Compression garments and sleeves for chronic lymphedema
  • Sequential extremity pumps and accessories for primary lymphedema.

Description

The Assistive Devices Program (ADP) in Ontario provides funding for pressure modification devices for individuals with chronic physical disabilities.

To apply for funding, you’ll need to be a permanent Ontario resident with a valid health card and a chronic physical disability requiring the use of these devices for at least six months.

You’ll also need to be assessed by a lymphedema team for lymphedema-related devices.

Here’s a step-by-step guide:

  1. Confirm Eligibility:

Ensure you meet the eligibility criteria: a permanent Ontario resident with a valid health card and a chronic physical disability requiring the use of pressure modification devices for a minimum of six months.

  1. Obtain Application Form:

Get an “Application for Funding Pressure Modification Devices” form from an ADP-registered authorizer or vendor. You can also download it from the ADP website.

  1. Medical Assessment:

The first time you apply, you’ll need to be assessed by a specialist physician. For lymphedema-related devices, you’ll need an assessment from a lymphedema team.

  1. Find a Vendor:

Locate an ADP-registered vendor who can provide the specific device(s).

  1. Complete the Application:

Work with the vendor and assessment team to complete the application form, ensuring all required information and documentation are included.

  1. Submit the Application:

Submit the completed application to the ADP.

  1. Await Decision:

The ADP will review your application and notify you of their decision.

  1. Funding and Procurement:

If approved, you’ll receive funding to help cover the cost of the device(s). You can then purchase the equipment from the registered vendor.
